Challenge

Breaking silos for better patient outcomes

Patients with multiple chronic conditions often receive care from different providers – general practitioners (GPs), hospitals, municipalities, and specialists. But their health data is scattered across disconnected systems, leaving patients to carry the burden of tracking appointments, medications, and test results.

This lack of shared insight meant vital information could be missed, leading to gaps in communication, repeated tests, unnecessary visits, and care that felt fragmented instead of coordinated.

Solution

A shared interface for cross-sector healthcare decisions

Together with Steno Diabetes Center Aarhus, we designed and developed Samblik – a system that integrated patient data from GPs, hospitals, municipalities, and podiatrists through the National Service Platform.
With Samblik:

  • Healthcare professionals had one shared view of a patient’s essential health data
  • Decisions were made faster and with more confidence
  • Duplicate tests and administrative tasks were reduced
  • Patients experienced a smoother, more connected care journey

Following a 2022 pilot with 32 healthcare professionals, a two-year trial was launched in the autumn of 2024 across three Danish regions – delivering faster decisions, easier access to information, and less administration.

Our Process

Dual-track development for maximum impact

We used a dual-track process, running discovery and delivery in parallel to move fast while reducing risk:
Discovery track:

  • We worked closely with healthcare professionals and patients to uncover needs, pain points, and opportunities.
  • We mapped user stories to identify high-impact features and define the product vision.
  • We created prototypes early to explore workflows and validate concepts.

Delivery track:

  • We developed working features in short, focused sprints, starting with those that provided the greatest immediate value.
  • We integrated continuous user feedback to refine functionality as it was built.
  • We built a flexible technical foundation to support future iterations and scaling.

By running these tracks simultaneously, we tested and learned quickly while still shipping usable features early – ensuring that the product met real-world needs from day one.
